In this rewarding role you’ll be optimising the provision of health and well-being supports for clients with disability (adults and children with complex health care needs), in their homes and within the community. You'll cover CPL and our sister company Cootharinga North Queensland.Working within the Cairns region + Mt ISA approximately every 6 weeks, you’ll conduct health care assessments, assist with complex care planning, liaise with internal and external health professionals such as Allied Health, hospitals and GP’s, and provide health education to clients and our support workers in the home and group settings.Selection criteria includes:AHPRA Registered Nurse with at least 2 years post qualification experience.A variety of Clinical experience and skills, including complex care planning, ideally in a community setting.Training experience (TAE qualification highly desirable).Great communication skills to liaise with clients, families and stakeholders.Great trouble shooting ability.Ability to self-manage and organise your days.Previous experience within disabilities (desirable).Be competent in the use of technology and IT systems.Hold a valid C class driving license.NDIS Clearance before starting.COVID-19 Full VaccinationPlease review the attached position description.Benefits include:Permanent, part-time role with flexible days.3 days per week.Travel in the Cairns region + periodic trips to Mt Isa.No shifts or on call work, no weekends.Competitive salary and packaging (up to $5000/yr tax savings)Autonomy to manage your own diary and time.Opportunity to work in the community where no two days are the same!Be an integral part of our Health Liaison & Clinical Governance Team.Want to make a difference to the lives of people with disabilities?
