In this rewarding role you’ll be optimising the provision of health and well-being supports for people with disability and complex health care needs, both adults and children across the age spectrum, in their homes and within the community.Working predominantly within the Logan & Redlands areas, you’ll conduct health care assessments, assist with complex health care planning, liaise with internal and external health professionals such as Allied Health, hospital teams and GP’s, and provide health education to our support workers & clients in home, centre based and group settings.You will work between our Capalaba, Hillcrest, and Beenleigh sites from a Browns Plains base.Selection criteria includes:AHPRA Registered Nurse with at least 2 years post qualification experience.A variety of Clinical experience and skills, including complex care planning, ideally in a community setting.Training experience (TAE qualification highly desirable).Great communication skills to liaise with clients, families and stakeholders.Great trouble shooting ability.Ability to self-manage and organise your days.Previous experience with disability (desirable).Competent in the use of technology and IT systems.Hold a valid C class driving license.NDIS Clearance required prior to commencement.COVID-19 Full Vaccination.Please review the attached position description.Benefits include:Permanent, full-time role.No shifts or on call work, no weekends.Competitive salary and packaging (up to $5000/yr tax savings)Autonomy to manage your own diary and time.Opportunity to work in the community where no two days are the same!Be an integral part of our regional & Clinical Governance Teams.Want to make a difference to the lives of people with disabilities?
