In this rewarding role you’ll be optimising the provision of health and well-being supports for both adults and children with disabilities and complex health care needs, in their homes and within the community.Working across the Rockhampton / Gladstone region, you’ll conduct health care assessments, assist with complex care planning, liaise with internal and external health professionals such as Allied Health, hospitals and GP’s, and provide health education to clients and CPL support workers in the home and group settings.Selection criteria includes:AHPRA Registered Nurse with at least 2 years post qualification experience.A variety of Clinical experience and skills, including complex care planning, ideally in a community setting.Training experience (TAE qualification highly desirable).Great communication skills to liaise with clients, families and stakeholders.Great trouble shooting ability.Ability to self-manage and organise your days.Previous experience within disabilities – desirable.Be competent in the use of technology and IT systems.Hold a valid C class driving license.NDIS Clearance before starting.Please review the attached position description.Benefits include:Permanent, Part-time role with flexible working hours.No shifts or on call work, no weekends, 3 days Part time.Competitive salary and salary packaging options (including a car).Tax benefits on salary up to $15,900 and $2,650 annually.Additional annual leaveAutonomy to manage your own diary and time.Opportunity to work in the community where no two days are the same!Be an integral part of our Health Liaison & Clinical Governance Team.Want to make a difference to the lives of people with disabilities?
